About a week ago, started acting funny, would dart around the tank and crash into everything once every few hours, sometimes swimming in circles, laying on its side or freefalling vertically. These few days shes not doing that as much but the skin around her head looks like peeling off and i can see a few scales on its body came off too.... What kind of disease is this? Will she survive?? Ive had her for about 2 years, no changes to diet or anything else

It's hard to tell but looks like it could be a very advanced case of velvet. Did you know a fine dust like covering on her before the skin started peeling away?

What size is our tank and what are the inhabitants? Do you test your water for amonia, nitrite, nitrate and PH? If so what are the stats?

There are over the counter remedies for velvet (if that's what it is) but it looks pretty far gone. In the early stages the fish will dart around the tank, flicking as if something is irritating them, as the disease becomes more advanced they'll quieten down and perhaps start developing secondary bacterial infections. I'm afraid by this stage it's too late and it's normally just hours or days before the fish dies.

The other possibilities are a fungal infection but normally you see white cottony wool patches on her. Did you see anything like that? Again, anit fungal meds are available. However I'm more inclinded to go with velvet. It is contagious and you'll have to check the other fish in the tank for any gold dust.
 
We need to know your water stats in ammonia, nitrite, nitrate, and ph.
I agree with scotterhchick as it sounds like columnaris as well as a parasite.
