What's Wrong With My Angelfish?

Yes it bacterial so you will need a bacterial med.
What are your stats in ammonia, nitrite, nitrate, and ph.

Looks like a case of Columnaris to me. As Wilder suggested get a good Bacterial medication, when I had Columnaris I treated it with Furan-2 and it cleared it up, but there are better meds on the market.

Bad water quality.
How long has the tank been set up.
How often do you maintain the tank.

The tank was set up almost 6 weeks ago but i transferred everything from my old tank into the new tank such as the water, gravel etc....

Also i transferred my old filter media into my new tank and ran it alongside my new filter to clone it.

The old filter media was only taken out a few days ago.

Isn't Melafix the best thing to buy for this kind of thing?

Right removed the old filter media to soon the tank gone into cycling again.
Don't do large water changes it just prolongs the cycle distressing the fish for longer.
Best to get it over and done with as fast as you can.

Melafix has it uses but not with columnaris if its a bad strain in can wipe a whole tank out.

What medication do you recommend to try and solve this problem??

When i asked on here how long i needed to run the old filter media with the new filter i was told around 2 weeks so i thought leaving it in for over 5 weeks would have been long enough!!!

Where that reading on the chart.

My API freshwater master test kit manual says that the Ammonia level may surge to 4 ppm or more and then fall rapidly as the biological filter becomes established.

It says that a nitrite reading of between 1 and 5 is to be expected and will eventually fall to 0.

It says that a Nitrate reading of 40 ppm or less is recommended for freshwater aquariums.

My Nitrate reading is ok, it's just the Ammonia and Nitrite that is not too good although it is not really high!!!!

I am using aquasafe and also i am using king british safe water everyday until the filter becomes established!!

Thats what I thought with your nitrate reading not being high, only have one apI test kit and that ammonia there charts are different.
I would say another two weeks or less to get them stats to 0, though ever tank different.

That's good, hopefully the stats will drop very soon, what medication do you recommend to treat the columnaris?
