Tetra Lost Their Fins

I have few community fishes (danio zebra, black tetra, neon tetra, guppies, albino buena tetra, pleco). In the night, albino tetra (orange) seems to have lost side fins and just moving with back tail, mostly upside down. I am not sure if any fish damaged it, it damaged itself or it got any other problem. I don't see fishes fighting except one danio chasing the other danio (I am guessing that it is normal). Can I do anything or it will die?
 
Sadly if he's upside down i would remove him from the tank, or put him in a container at the top of the tank, as he sounds like he's not going to make it.
Can you also post stats in ammonia,nitrite,nitrate,and ph.

This is a 1+ month old aquarium. Its cycle got balanced after some nitrite spike around 10 days ago. I don't have ammonia test kit, but quickdip 5in1 shows nitrate level 40ppm and nitrite level 0.5ppm (or less).
 
Right need to get that nitrite reading down with a water change.
The tail problems could be from the cycling tank, bad water quality if you have seen no fin nipping.
