Tetra Dying Of Something - Any Ideas

Hi,
 
One of my ember tetras is in the process of dying of some sort of fungus or bacteria or something.  One Dwarf Rasbora died a few days ago but I did not notice the same symptoms.  All other fish seem healthy and active, including my two baby rasboras.
 
Tank Size: 16 Gallon
pH: 7.2
ammonia: 0
nitrite: 0
nitrate: 10
kh: no test chems for this
gh: no test chems for this
tank temp: 78
 
Fish Symptoms:
Ember tetra started to have what appeared as white fuzz/mucus/? on the body.  The normally clear parts of body (like near tail, etc) looked white (inside), started out trying to stay away from other fish near bottom, today almost dead hiding in plants near top of tank.  He has been sick for approximately two days.
 
Volume and Frequency of Water Change: Every 10 days or so 1/4 to 1/3 water change
 
Chemical Additives:  Sea Chem Prime at water change, FloraPride 0-0-3 maintenance
 
several live plants (Amazon swords, Lutea), Large driftwood with moss, small drift wood with moss
 
 
Tank inhabitants:  6 Dwarf Rasboras, 6 ember tetras, 2 assassin snails, 1 algae shrimp, 2 cherry shrimp
 
Recent Additions:  My 10 gallon tank sprung a leak about a month ago and I moved a couple of plants, my cherry shrimp, the small drift wood and 2 baby Dwarf Rasboras.  Purchased one large Amazon sword about two weeks ago.  Removed 6 algae shrimp about two weeks ago.
 
Exposure to Chemicals:  none other than mentioned above
 
Picture:  added but does not show it as good as I hoped.
 
Any ideas?

The tank has been fully cycled for over a year now.

Was it more like a fuzz like fungus or more like a faded patch (possibly something like Neon Tetra Disease).
 
White fuzz usually is Ich, sort of looks likes it; but I'm not too sure if this is the same.

To get rid of Ich, you need to increase the water temperature slowly to 86*F or 30*C, do a water change (make sure you are vacuuming your substrate) and increase the airflow in your tank.
 
There are medications but they don't work nearly as well than the above.
 
A couple of years ago I had two Bettas that had ich and it didn't really look like this.  I'm not saying it couldn't be ich.
 
It looks fuzzy to me.
 
velvet disease can look as white as ich sometimes, usually its more of a dusty gold eefect but depends on the strain :S
