Sick Puffers

I have 2 puffers in a 50 gal tank. I noticed a month ago the smaller puffers belly would turn grey when on one side of the tank and as soon as he would swim to the other side his belly would turn white. The larger puffer's belly would always stay white no matter which side he was on.For the last week the smaller puffer would lay at the bottom of the tank for long periods of time. Also the bigger puffers belly now is turning grey when on that particular side just like the other one. Just today the smaller puffer has spent most of the day floating at the top with his mouth wide open almost out of the water????? Wats goin on with these little guys? Ive had them for over a year with no problems. All the water tests i ran checked ok..Amonia,PH,nitrate..Any ideas? Also they are panting alot. I have my water filter sending water from above the water line to oxyginate the water more. Ive always had it this way with no problems till now.

you need to provide a lot more information, puffer species, water stats, routines, what else is in the tank, any new additions to the tank...etc etc
 
They're GSP's, the salinity is around 1.016, I recently tested for amonia and nitrates and both tests were good. I do weekly water changes. I added a mono about 2 months ago, they get along fine. I feed them once daily mainly shrimp i buy from the store and i feed them snails also. One puffer is almost twice the size of the other. I was thinking the little one might have worms.
