Red Severum Again

Hi, I am in need of help with my red severum still and am at the point of screaming.

I recently did a treatment of sera nematol to treat for the callamanus that the fish was suffering from. I am yet to do the repeat three weekly dose that is required in a few days. There has been no sign of worms since and he is putting weight back on.
After 2 days of treating with the nematol I performed the required 80% water change and noticed that the severum had a slight issue of ICH. So I have had the temperature at the 30c for two weeks and ran a course of protozin with daily 50% water changes for the past two weeks. The white spots disappeared..........however

After all of this I still have a fish that is flicking and rubbing against sand and ornaments, occasionally lethargic and pretty much constantly flickering its tail and fins as though something is itching.

I am now pulling my hair out everytime I see the slightest fin flicker.
Please help
 
Size of tank in gallons or litres.
How many fish and which type.
Water stats in ammonia, nitrite, nitrate, and ph.

When you preformed the water changes did you add the correct amount of med back to water removed.
Did you do the 50% daily water changes during the med course.
