Really Sick Clown Loach

For the past 24 hours I hav been keeping a very close eye on one of my small clown loaches. I had hoped that he would get better, but instead he is just the same if not worse. What it is, is around his mouth is really deep red colour, just like his mouth is actually bleeding. I first noticed this wen I was inspecting him because he was floating at the top of the tank. He is still floating and his mouth looks really bad. I really need help!
 
Size of tank in gallons or litres, how many fish and which type, water stats in ammonia,nitrite,nitrate, and ph.
The bleeding is it beneath the skin or on top, also any fungus growing on the mouth area.
 
there is no fungus at all, he is kept in a 240 litre community tank with around (not exactly 20 tank mates. you can't tell if bleeding is on top or underneath, and all of my water stats are perfect. I checked them yesterday when I first noticed it. Also he is breathing really fast.

I do a 35% water change every week.
 
Can you get a pic up, also is there any redness anywhere else on the fish, and could he of hurt himself on something in the tank.
 
There are things that he could hurt himself on but not to the extent that he is, I appologise as I cannto take a photo as my mum is in India on business and she took the camera :( But there is no redness anywhere else on the fish.
 
he has started floating upside down now! :( I don't think that he has much chance. I dont have any meds that would be suitable right now and I dont think that he will last untill tomorrow. He isn't swimming or anything! I fear the worst.
