i know of rays jumpin out sumtimes, to explore, but never "crawling" out. most of the time they just stick their nose out, cuz they like the feel of the air. where i work, we've never had any problems with the rays, so my guess would be tank size as well. might need more room
 
water params and every thign were fine all checked and double checked.


As for tank size they were only small little ones in a 130 gallon tank. the water level is fairly close to the top and they seemed to edge there noses out constantly.
 
there u go then. if u have a water level only a few inches from the top, they will be prone to sliding out. at the aquarium i work at, we have round stingrays in a 200 gallon tank, and the water level is a good 7 in from the top to keep any rays from hopping out. same in our outdoor bat rays tank, which is a good few feet deep, water level is good foot from top.

remember rays r bottom dwellers, so the water levels dont need to be very high, but more of a good width and area for them to swim and be buried.
