Please help, my platy is very sick :(

Tank size: 15 Gallons
pH: 7.6
ammonia: 0
nitrite: 0
nitrate:
kH: 80 (4.48) - 8 drops added to sample of water
gH: 120 (6.72) - 6 drops added to sample of water
tank temp: 80

Fish Symptoms (include full description including lesion, color, location, fish behavior): lays on the bottom of tank, not eating, not active, loss in color, top fin is down and other fins are closed, breathing heavily. Also mouth appears to be somewhat deform??

Volume and Frequency of water changes: change every 2 weeks max.

Chemical Additives or Media in your tank: use aquaplus and waste control, 1.5 capful as recommended dosage per 15 G tank.

Tank inhabitants: have 2 live plants and 5 other platty fish.

Recent additions to your tank (living or decoration): none.

Exposure to chemicals:

Digital photo (include if possible): not available.
 
Not good take the fish out of the tank even if you issolate it in a container at the top of the tank.
When you say deformed do you mean a bent spine.
 
Take it out to avoid spreading disease to the other fish or so that the other fish don't bother her? I don't have another tank to separate them. By putting her in a container, wouldn't that be worse for the sick fish as they wouldn't have the proper air, etc.?

I don't think she has a bent spine. What would that mean? What I meant by deformed is it almost looks the top part of the mouth has caved in?
 
The isolation is mostly so that it does not infect the other fish. Unfortunately, it doesn't look good based on the symptoms you described. I'm sorry to say that I wouldn't be surprised if your fish doesn't make it.

Does there appear to be any kind of spots or sores around the caved in mouth? Or anywhere else on the fish's body?
 
Any idea what would of caused this or what the fish might have? I'm ammusing poor water conditions is the cause, but what can I do to improve it based on my readings I gave above?
 
Really your water conditions don't look bad. The pH might be a little high, but that's only bad if it's something that fluctuates. If your tank always has that pH then your fish should be acclimated to it and won't be harmed by it.

There's no way for me to tell what's wrong with your fish because all of the symptoms you've described (besides the deformed mouth) are just due to stress. That's why I need more information about it.

Has the fish always had a deformed mouth? Does it look like a sore or are there any spots on it? Any other abnormalities on the fish (spots, protruding scales, etc)?
 
The reason why I said issolate her she dying.
How is she now.
