Not Sure What To Do

I have a black skirt tetra that hasnt been doing well for about a month. I now regret not doing anything about it. I thought it might just be old age because he is over 3 years old. but he has been swimming on its side and now swimming vertical. One of his eyes seems like he might be blind, its kinda cloudy. But he is still trying to eat. none of my other fish have gotten sick so thats why i think it could be old age. Anyone have any ideas or what i can give him? All readings for water are normal and its a 55 gallon tank.
 
How many fish and which type.
Water stats in ammonia, nitrite, nitrate, and ph.

Swimming on there sides can be many things from bacterial to internal parasites.
Can he maintain his balance.
What does it look like when he goes to the toilet.
Just check the fish anus to see if its enlarged or red and inflamed.
Is the fish bloated.
What do you feed your fish.
Cloudy eye is a symtom of a desease not a desease in it's own right.
 
Black widow tetra can live up to 5 years.

Its a 55 gallon tank. GH is 120ppm, KH is 80 ppm, ph 7, Nitrite 0, Nitrate is high at 80ppm, ammonia is a little over 0.
I have 7 black skirt tetra, 7 tiger barbs, 2 rasboras, 3 zebra loach, 3 panda corys, 1 guppy, and just added 1 clown pleco today.
He isnt inflamed at all and i havent seen him go to the bathroom. his balance isn't great and he struggles to swim to the top. Sometimes he rests on the bottom but will swim around when feeding time. I usually feed them flakes.
 
Your nitrate high.
When did you last maintain the filter.
How often do you do a gravel vac and water change.
Do you over feed your fish.

Need a better diet of frozen foods and veg.
is the fish bloated.
Does he fall back onto the substrate.
Need to do a water change if you have an ammonia reading.
 
He's not bloated. I changed the water and cleaned filter about 2 weeks ago. But i got rid of my common pleco and algae started to build up. Im hoping my new clown pleco will clean the algae.

 
Do you use a gravel vac.
When did you last rinse one of your filter sponges in old tank water.
If you can issolate I would try a bacterial med.
