Losing Scales

I have added three new fish(clown loaches) to my aquarium that had a tetra and the loaches have all died and now the tetra seems to ne losing scales. I see scales floating in the water. Any ideas??

Im not sure about the loaches but scale loss can be caused by injury: fighting with other fish, sharp rocks.
Are there any little white grains on his current scales. If so that's "Ich" get some medicine i guess.

Scale loss can be due to parasites too, so keep your eye peeled, plus clown loaches are prone to whitespot so look for grains of salt on the fish.
