looks like a worm comming out of his mouth is he badly ill? should i move him to another tank

Did you feed him any worms ? Are you very sure it's a worm and not just a plant root or some debris ?

well it's got longer so it's comming out i think only just had a dead sword tail in the tank and when i looked closer looked like somthing had been eating the fishes insides so could it be part of the dead fishs insides?

Been doing water changes and got.....

ammonia down to 0mg/L
Nitrite 2.0mg/L
but nitrate is really high 40 -80 mg/L
tested my tap water and it has a nitrate level of 20 mg/L so whats the best way to get it down becuase some of my fish are not look that good

Tanks well on its way to cycling, but you will need to carry on with small water changes.
