Large Open Wound

Any ideas please

my golden gourami has a large hole which looks as though another fish has attacked it it seems to be healing over with the use of melafix but i dont know if it will ever totally repair itself. the fish is my oldest at six years and is kept in what has been a good community tank although i put the damage down to my to kissing gouramis. any one else had anything like this

Size of tank in gallons or lites.
How many fish and which type.
Water stats in ammonia, nitrite, nitrate, and ph.
Where is the open wound.
Anything sharp in the tank.
Can you issolate the sick fish as opens would really need treating with an internal bacterial med.
