Injury or Illness?

I have a lab. caeruleus that has a small hole behind his right side fin. (sorry, I can't remember the exact name.

Anyway, its the fin right behind the gill.

Is there an illness or disease that can cause this?

Or, is it more likely an injury?

What is my post infeccted with West Nile or something?

It is most likely physical. Is it on the body behind the fin, or is it actually on the fin itself?

How big and deep is it? Is it discolored in any way? Any growth or sign of infection? Is the fish otherwise acting normally?

It was right behind the gill, above the fin.

It had no coloration. (beside the normal yellow of the fish.)

He passed away in the hospital tank whiel I was at work yesterday.
