Identification Of Problem Please!

This has been going on for a long time and none of the normal treatments have worked.

The fish in question has been in my care for the past three years and god willing I hope to have him for a lot longer. There is a due of problem that I am looking to have identified. I have labelled both problems in the pictures below and I shall discuss them in two parts.

A: To me this looks like ich, its visible salt grained sized dot on the gills. But if it is ich none of the treatments that I have used have had an effect. (treatments being, intrepet med's for ich and salt treatment). What brings me to believe that it is not ich is that the repeated treatment of meds and salts haven't worked and that the spots are a constant feature. Surly with the life cycle of ich the spots disappear when the eggs have been released. This is not the case in this situation as the same spots have been in the same location for the past 3-4months. There are only one or two other spots on the pectoral fins but no other signs of ich anywhere else on the fish. Can anyone please shine some light on what these might actually be? and if there is a treatment. All other fish are spot free!!

B: The next question is in regards to the pectoral fins. From the very beginning this fish has had poor quality fins. I am not sure if it is occurring naturally, if it is mechanical abrasion or it is some sort of bacterial infection. I have tried treating with intreped's anti-fin rot medication but to no affect and the fins still remain pale and moth-eaten at the ends (last 2-3mm). Also there are a number of raised white bumps along the front of the pectoral fin. I also use Stress coat with alovera so any physical damage should be healed nearly immediately so it can only be genetic or an infection of some kind.

The fish has never shown any sign of ill effects, ie. gasping, rubbing on substrate, listing. The fish feeds well and it highly active. Any information or suggestions would be greatly appreciated.

Water changes: Every 5 days at about 30%.
Occupants: 1 x ranchue, 2 x moores.

Those are breeding tubercles and mature males get them when they are ready for breeding.

He is about 4.5-5 inches in length. I am very tempted to take him and weight him, then do it again in a couple of months just to see how much weight these guys can put on.

It is a shame that he still has part of his dorsal fin, but it doesn't bother me in the slightest. Great fish with a great character.
