Ich/white Spot? Or Could It Be Acne!?!

Not an emergency as such, but....
I have a white molly who I noticed had a tiny white lump (approx 1mm) on her anal fin and another on her body just below her dorsal fin. Apart from those 2, she is perfectly fine, no other symptoms.
I havent added anything to the tank in the past month and all the other fish are fine too.
Water parameters are ammonia 0ppm, nitrite 0ppm and nitrate 40-50ppm (tap water is 40ppm)

I know what ich is, and have bought some anti white spot and parasite medication - contains formaldehyde 5000mg and malachite green 47mg per 100ml.....so....do I treat or not? And, will all my fish be ok? (Theyre in my sig)

How long has she had just the two spots? Seems rather large for ICH which is the size of a grain of salt. I almost want to say wait and see.
