I Think We Might Have Ich

Today while watching my fish I noticed one of my striped fish going towards the rocks and flipping really fast against it. He is doing it over and over. I started looking at the other fish and my fire fish has a small grain on him and two other fish have 1 small grain each the chromis won't stay still for me to look. I was told the six line wrasse could eat ich off the other fish but he too has a spot on him. It is possible that it is just something from the tank on them but the firefish has had the same spot for over an hour. What are my options here I have ich med but it doesn't say much about instructions we just added two zooanthids will ich med kill coral. Quarantine will not be possible as we have over 200 lbs of live rock and it is impossible to catch the fish. any other ideas

Could be sand embedded in the fish's mucous. Otherwise, could be worth it to invest in a UV sterilizer or cleaner shrimp.
