Holes In Leaves

I did post a while ago when my well planted tank suddenly started disintegrating. Rather than my usual small amount of plantamin I added something like the full dosage for the water. Within days the plants, especially the amazon swords, were getting holes along each side of the mid rib of the leaves and from there eventually turned into skeletal forms and died off (actually the roots seem ok and they occasionally put out a new leaf but it soon goes holey). I have been doing regular 20% water changes (did a 40% first off) and remove leaves before they break up but even new plants added are still doing this. Its been weeks now and I haven't added any further plantamin. Someone recommended flourish and i will switch to this, but should I add it when even new plants are going the same way?
hope someone can help. The base is sand then gravel, I have co2 added but the lighting I know could be better, but it was all going really well before this happened. The Java ferns seem ok.

I would start using the flourish asap, as it sounds like a nutrient deficiency and flourish is an excellent trace mix. I've grown swords is just over 1WPG so I know they are a hardy plant. Do you use root tabs? Some sort of root nutrient nutrient would probably also help.
