Hole On Leaves; Torn Stems

Hi all,

I have noticed round holes appearing on the leaves of my waterlilies and anubias. The holes are in the middle, so I don't think they were made by fish nipping on leaves, since they nip at the edges (if they even do that). I guess there is an organism that is causing the tears. Does anyone know what could have caused these? Also every couple of days I see a lily leaf floating around with a stem that is torn and shredded (kinda). I was wondering if my fish are doing this or the mysterious organism(s).

sorry notg2009, that to be looks like a deficiency of Nitrogen, i had the same this start to happen to my Lotus. I upped my Nitrogen doing and the lotus picked up.

what ferts and any C02 being dosed? Also what is your lighting?

I used to use tetra florapride but recently switched to sachem flourish which should be much better. There is always ammonia and nitrate available in my tank but nitrite is zero always. As for co2 I don't use any since I was warned about dosin unwanted co2. At the LFS I was told that Flourish should take care of the matter but I don't trust them fully ;) I use LED lights for lighting. They are not very powerful but all my plants should be ok in low light settings. Any idea what is wrong.

EDIT: I use Amquel plus, ammonia detoxifier to remove ammonia, nitrite, and nitrate from the water. While these compunds should still be available to the bacteria, I'm not sure if plants can use them as well.
 
anyone knows if Seachem Flourish takes care of this problem?
