Hole In The Head! What Do I Do?!

My dwarf gourami seems to have the hole in the head disease. There is a spot that I've just noticed right between his two eyes and suspect that he has got the disease. What causes this and how do I get rid of it?!
 
Can you describe the spot in more detail it dosn't always mean hole in the head.
Columnaris spots are greyish white with a tinging of red sometimes around the edge.
Or some spots have a red centre.
 
Ulcers look pink or red with a circling of dead white skin around the edges.

I cant get a good picture of the spot on his head because it's kinda tiny. It just looks like a plain white sunken dot. I've also noticed that since the dot appeared he looks like he is having trouble breathing. He's constantly gulping in water and either floats at the top or stays towards the bottom for a long time. I've had him for about 5 months now and this is the only trouble i've run into with him.
 
He sounds liike he dying to me sorry.
You can get columnaris spots which are bacterial they are quite hard to get rid of.
Never heard of a gourami with hole in the head.

That may be the case, but you still need to sory out WHY the fish got HITH in the first place, water quality is a starting point to look at

I never ever did find out what this was but it happened about 6 months ago, I can't even remember if I treated him but I do know that I changed the water every 2nd day and he recovered. He's still doing fine and doesn't even have a scar.

yeah looked just like that.. i checked the water quality at the LFS and they told me that the water was in great condition. I did water change outs just like you did on every other day and still no luck. i dont think it was HITH after collaborating with everyone from here and the LFS and rather columnaris.
