Hole in Head looking better

Hi. My fish and I give a big thanks out to those who helped with our HITH panic the other day! I treated my tank yesterday and today with Hex-out. My fish store guy told me that the holes would begin to appear blackish on the edges and may even bleed as blood supply returns to damaged tissue. Im starting to see a little of both of those signs (not actually bleeding, but pink and I can see small vessels in the worst of the holes now).
My questions are
1. how long do i continue treatment? The hex-out instructions say to do a 50% water change 3 days after symptoms are no longer visible. well, my fish store guy also said the holes never actually heal, they just stop progressing. so how do i know when to stop treatment? (i also have 3 poor snails sitting in a bucket next to the tank waiting to go back in! they also seem to be sloughing a lot of tissue from their shells but it was my impression that i was not to medicate any inverts - opinions? i certainly dont want them to reinfect my tank!)
2. where did this come from and what do i need to do to prevent recurrence? i understand it frequently recurrs, but i need to do my best to find and eliminate the source!
