Help - Yellow Tang Problem

I have a yellow tang that is laying up against the side of the aquarium on the sand. It is breathing but not swimming at all. There are apperance issues as it is still bright yellow and no pale or spots. Last night it was swimming oddly. I have had this fish for only 4 weeks and have not had issues. It ate fine yesterday.

I also have 2 clowns, 2 blue velvet damsels, 1 valentini puffer, 1 forktail blenny and all are doing just fine. I also have a black longspine urchin, 2 hermit crabs and several snails.
Tank is 75 gallon with 100lb LR. Water quality is good with all areas but nitrates are a bit high at 30. The inverts are doing just fine also.

I feed them 1/2 frozen formula 1 and 1/2 formula 2 tabs each day and supplement with seaweed.

What should I do? Is it dying?
 
Please help anyone ... When I touch it with the net, it starts swimming but soon finds a place to lay down
 
you should ask this to be moved to the marine section...either in fish or marine chit chat. You'll get better responses that way. Just pm a moderator or two. you can go to the board index to see who's on that you can pm right away in hopes that they see it before they sign off.
 
Your nitrate reading fine.
What your tap nitrate reading.

Is he bloated.
Can he maintain his balance in the water.
Does he try to get off the bottom of the tank only to fall back down again.
Is he resting to onside.
Does he swim on his side.
Does he turn upside down in the water.
What does it look like when he goes to the toilet.
Check his anus to see if its red and inflamed or enlarged.
What do you feed the fish.
