Help Sword Tail dying

I just bought some new plants, and all are doing well except for the swordtail. Holes for in it, and it seems like the plants just disappears. Can anyone tell me what is going on.
 
I have the same problem! When I started my tank a couple weeks ago, I added the Amazon Sword. I also added some Flora Pride liquid fertilizer. I recently added nutrient root tabs under the plants. But there are still some holes in it.
I'm quite sure it is the lighting. I have a 24" 19 watt (I've checked the stores for alternates but all they have are 24" 20 watts...same difference). I have a 15 gallon. I'm quite sure I have the nutrients needed especially for an easy plant like the sword. I have six fish so far and I am quite sure there is enough CO2. So I would check lighting. It seems to be the most important factor for the plant because the recommended is 2 to 3 watts per gallon. I only have a little more than 1 watt per gal. :/
 
might be the lights?? you'll need atleast 30watts fullspectrum lights

I have 3.5watts per gallon and a combination of both fullspectrum and special lights but mine is still doing it? My other plant are growing very very well, I added root tablets and fertilizer, mine is almost 8 month old and some leaves is growing well other not.
