Help - My Convict Cichlid Is Losing His Stripes !

Just noticed that my male convict has become much paler and lost his zebra stripes. He's almost white now with only one dark stripe. Also noticed red on his tail on the narrow bit where it joins his body . Could he have hurt himself against something like the heater, did this once before on a rock which I removed.. He used to be handsome with very dark zebra stripes until recently.

Poor fish has been through a lot in last few weeks. Mystery toxin/oxygen lack following 40%water change killed his mate and all their fry. Almost killed him too but managed to save him by moving to hospital tank. Tank was cleaned throughly and gravel sterilised by boiling but added a little water from other tanks for bacteria before putting him back in.

He's fairly active and healthy now, apart from colour loss. Maybe because he's alone in tank and no longer breeding?
 
What are the actual water stats in the tank? if you sterilised everything and didn't add any mature media back to the tank then the bacteria would have been lost,
Just adding water from another tank will do little to help restore stability as only trace amounts of bacteria are found in the water
