Graumie Illness?

I have had a freshwater tank for about 5 months and have had 2 graumie die. They all looked the same before death, bloated, flaky scales and scar looking apperance. I have one right now showing the same signs and i'm looking for insight. All have been dwarfs. Help please
 
Size of tank in gallons or litres.
How many fish and which type.
Water stats in ammonia,nitrite,nitrate,and ph.
Being bloated can be swim bladder, dropsy, constipation, and parasites.
Can the fish maintain it's balance in the water.
Flaky scales bacterial to parasite, what colour does the flaky skin look like.
Any laboured breathing or flicking and rubbing against objects in the tank.
 
i can answer some of those but not all.
30 gallon tank mostly tetras, neons, platties and other graumies.
right now the fish at times is not balanced but on the whole usally is, brathing looks very labored, flaky scales are pale blue/white. this is a terqouise graumie. looks like bite marks??? staying close to the surface.
 
Can you issolate him he's dying sorry.
 
this morning i found my fish dead. half of it's body was white no longer blue mostly the belly and around the face. It makes me sad because i really like the graumie and its color. If you can point me into a direction that may help me prevent this from happening again i would appreciated it. This fish i had for about 2 months and was doing great 4 days ago we added new fish to the tank and then my graumie dies what gives???

Gouramis are very prone to bacterial infections, plenty of veg and frozen foods in there diet.
