Tank size: 40 gal
pH: 7.0
ammonia: 0
nitrite: 0
nitrate: 10mg per l
tank temp: 28 C
Fish Symptoms (include full description including lesion, color, location, fish behavior):
Had a few die off. All one at a time. No symptoms will show, then suddenly they will lay down on the bottom and die within a day.
One fish has shown sides of possibly a bladder problem (swimming at weird angles) but seems to in general get better awhile after eating. Going to try to treat him individually and have read that a pea may help things in that department.
I've had a swordtail, an angel, and a sailfin molly all die with no obvious reason in the last month+
Volume and Frequency of water changes:
25% every 3-4 days, 50% if i see someone not looking good.
Chemical Additives or Media in your tank:
UGF, no extra filters. No chemicals added.
Tank inhabitants:
1x Pleco (quite small still)
1x Sailfin Molly
4x Zebra Danio
1x Dwarf Gourami
1x Sailfin Platy
Recent additions to your tank (living or decoration):
I haven't added anything in over a month as once my first fish died I wanted to see things stay healthy for awhile, which it hasn't. The last things I added were a pair of plastic plants and a malasian log probably 1.5-2 months ago.
Exposure to chemicals:
1 tbsp of aquarium salt per 5gal.
I have read mixed reviews on this, but am still doing it so far.

Not much to show since anything there is currently healthy.
I did not see any fungus or anything of the like built up on any of the fish that died. The swordtail did look to possibly have a bit of white fuzz last night, but I could not see anything when I woke up this morning though the fish had died during the night.
When I saw the fungus I had planned to head out this morning to get some medication and toss him into a hospital tank but he didn't survive long enough. Prior to last night he was one of the higher energy fish in the tank.
I started up this tank originally back in october. The cycle went fine, but things have just never been 100% stable.
Any thoughts / advise would be greatly appreciated.
