Fish Keep Dying

Hi there...
 
Me and my boyfriend have just started a new tank.. We have had it for about 3 weeks and our fish keep dying due to cloudy water..
 
We have 2 small filters in the tank, an air stone, a plastic plant and a decorative tortoise shell so that they have a place to hide in, we also have coloured gravel and a heater..
 
We clean the tank weekly and also add "aqua-safe" but yet 2 days later the tank keeps going cloudy and our fish just keep dying..
 
We have no idea if its the gravel causing this problem or the fact that we do not have a filter under the gravel as I've been told that, that is our problem..
 
Please help ASAP

You need to read up on cycling. Your tank is toxic to fish because it is not cycled.
