Fin Rot, Bad Water Test!

Problem number one:

My Betta has HORRIBLE fin rot... Literally, one moment he is fine, I look back after a few hours, and he's horrible looking. It happened SO quickly, I'm going to go get some medication today and now he's in his own containment, just in case.

Number two:

Found the reason for fin rot, HORRIBLE water...

PH: 6.4
Ammonia: 0.50
Nitrite: 5.0
Nitrate:0

How can I remedy this?
 
How long has the tank been set up, and do you keep any other fish with the betta, if so can you name them and how many.
 
The tank has been set up for maybe around a month... As a rookie at this, I made plenty of mistakes...

I didn't cycle. Wasn't worried though when I learned what it was, many things I read said Bettas were good cyclers?

Before I found out about cycling, I got some tetras and some loaches, big mistake, all are gone but were cleaned up properly and etc.

So, now, it's just him. Yesterday, before I left for a few hours, he was very upbeat and etc.
 
Just do small water changes as the tank will take for ever to cycle, good luck.
