I don't know about inches, but I do know that about one pound per gallon should be sufficent.
And try to slope it from the front to the back to give the impression of depth.
 
My stuff is between 1.5 and 2 inches. It slopes down, and is a bit wavy at the front.

For the laterite, the box says 1 oz per gal. If you arent going helvily planted, you could get away with 1 box scattered where you will most likely be planting. If you have lots of hardscape, then avoid putting any there. If you think you will have lots of plants, then I would go for 2. It will save you work in the long run.
