Dying Plants And White Fluf

hi i seem to be having a problem with my plants some of them seem to be dying a few have started dying off from the tips so i have cut away the tips in a hope to save them plants, my amazon sword plants are turning a yellow colour with brown marks all over them. and i cant remember the name of them but they have begun to die off from the centre of the plant turning from green to yellow.

there is also like a white fluffy substance on the base of the stems of the plants any idea what that is? possibly some sort of fungus? i cant get any pictures as i have just sucked up the fluffy stuff while doing a quick water change. there is also a strange planty kind of smell coming from my water aswell

a friend of mine has just told me that they might be dying off because they are having some sort of reaction to the sand? i used Tetra Complete Substrate then a 2-3cm layer of sand to cover it could the sand have anything to do with it?
 
I'm not an expert in this but I had swords with the same problem except the white fluff part. Did you trim the roots? Once I trimmed some of the old leaves still died but after removing them new leaves and stems grew from the base. For the white fluff I have no idea but to me it does sound like a fungus, I'm sure a more experienced member will clarify it soon.
