Dying Plant

Here is a pic of a small sword plant that my girlfriend has in her tank. 10 gallon tank, 1.5 wpg, no Co2, Lighting 12 hrs/day. Other plants include hornwort and wisteria.

Can anyone help out as to why this plant is dying and what to do for it to salvage it?

Looks OK to me. Do you use root tabs? I have the same sword, and I have root tabs and they love them. They feed via the roots, so youll need to add something to the substrate.

Whats on the leaves appears to be algae. A quick brush off in a seperate bucket, and then a wash down should remove most of it.
