Well it looks like I'm encountering my first medical condition since dealing with fish. This my Dwarf Gourami, I just noticed today that a patch of his skin looks like its gone right above his eye, and a little bit from his head right before his top fin starts, and the first part of his fin is black (not quite sure if it was like that before or if it matters). Not really sure whats going on here.

NH3- 0
NO3- 0
NO4- right around 10
Temp.- 78 F
Ph- about 6.4 (not sure if it has always been that low, I've been a bad boy and never have checked it  )
Tap Ph- about 7.2
55 Gallon Tank Ph (which uses the same water of course) 7.0

Tank mates are in the sig. He is acting normal, eating and swimming normal. None of the other fish look affected. Him and the other Gourami will pick at each others back fins every once in while, but nothing serious. Neither one of them gets over bullyed. I do a 15-20% water change once a week, last one was on Sunday. Don't use any chemicals besides regular dechlorinator. I feed them freeze dried bloodworms and flakes once, someimtes twice a day.

Anyone know what this is?

Right can you describe the patches are they fluffy, gouramis are prone to bacterial infections, does the patch look like if forming like a saddle along the back and beginning to come down the sides, any laboured breathing.
Black patches can be ammonia burns that are now healing, how long have you had the sick fish.
