I have recently had many fish die from what appears to be the same disease. They will be a little less social than normal and then they begin to sit on the bottom gulping for air and when I move them to my hospital tank to treat them they usually die within 2-3 days. I have tried many different medications and none have been able to save the fish. The ones that have died is a female german blue ram, male and female steel blue appistogramma, and discus. I do weekly water changes as well as ideal pH and water conditions. My tank is planted and have air stones and a UV sterilizer. Any ideas what this disease may be and how to keep future tank mates from dying of this disease? 
 
Is your hospital tank cycled? If they are only dying once they are moved to the hospital tank, they could be getting ammonia poisoning, worsening the already existing disease. Another problem might be the medication, if you are treating them for something they don't have it could be harming them. I'm not sure what they initial disease is, but I'm sure someone on here will be able to help.
 
The hospital tank is cycled. Typically the medication used is a general medication, not for one specific disease
