I agree we need more info.

Size of tank in gallons or litres.
How many fish and which type.
Water stats in ammonia, nitrite, nitrate, and ph.
The make of the test kit you are using.

Has the fish lost weight due to not eating?
What does it look like when your fish go to the toilet?
Is the anus enlarged, or red and inflamed?
is the tummy sunken in?
is the spine bending?

You replyed as I was typing a reply out.

What your tap nitrate reading?
Do you have live plants?
What's the make of the test kit you use?

Your water change is weekly, how much do you change each time? We would expect to see some nitrAtes unless you were very heavily planted. Even then I suspect there would be trace amounts. With all the fish in the tank are there any that are showing signs of breeding or aggression towards the discus? Even the other discus showing any signs of aggression.
