Difficult Ich Case- Please Help!

I have a dwarf gourami in a five gallon tank. I plan to move him to a twenty gallon mixed community tank. Problem is he has a reoccuring case of ICH that I just can't seem to clear it up. He had his first outbreak months ago. I raised the temp and added salt and it seemed to go away. Now it's back, which means he must have still had some level of infection this entire time (no new fish have been added to his tank in that time period).

I raised the temp again and added salt, but five days later he'd still clearly infected. This fish HAS to move within the next two weeks, so I really need to get this thing licked once and for all. Any suggestions for meds you've found effective on stubborn cases? I didn't want to go that route, but the salt and higher temp just isn't working this time.

This is my favorite fish and I would be endlessly grateful for any advice.
 
I have found that protozin by waterlife worked for me. As preparation it helps to get any suspended solids out of the tank with a good percentage water change and reduce feeding to a bare minimum then the meds are working on the parasite, you have just enough time to complete 2 courses should it be needed, but I only needed one

It doesn't look like this is sold at Petsmart- any good places to get it online? I don't mind having it rush shipped.
 
I also heard that the medication by King British is good.

Agreed about water changes.. I would personally increase the frequency and volume of them from now until you start on the medication course too so that a 80-90% one would not be too much of a shock. Also vacuum the gravel well before starting on the medication course.

Because his tank is so small I already do a big water change (greater than 50%) every two days, and have done so since the start (he'd been in the tank about 8 months). I also gravel vac three times a week. So he's well used to water changes and such...I started it when there were three fish in the tank (the other two have been moved) and it's probably more than I need to do at this point.
 
Look for a product called Rid Ich. This will beat any ich meds found in the UK, due to the need for a veterinary prescription in that country. A link to effectively treating ich can be found here.
