I think I would end his misery now.
Once fish stop eating there really unwell.

No he's not been in the filter he's too big to have gotten into there. Its been a very gradual thing but over the past 6 months its grown alot worse, I wasnt too concerned as he seemed happy enough but now he seems unwell I think its time to let him go

Any ideas why he would have grown like this? Is it something that i've not spotted or is it just one of those things?

ETA: Theres also a browny coloured slime keeps forming on the tank in the past week, even after cleaning the tank & doing 25% water change its reappeared after a couple of days?

Did you preform the awful deed.
Does your tank have brown algae.
I remember when I kept goldfish the tank glass was more slimey.
