Dead And Alive Plant?

I have a plant that dies and all the leaves fall off, then come back alive and grow stems and leaves , am I doing something wrong or is it dumb(or is it the amount of waste in the tank).

Does it die again and the leaves fall off again then grow back AGAIn and grow stems and leaves AGAIN. Or Is it only happening once?

If its only once, Its usually for plants that are sold Emerged growth, Submerged them and they will melt and grow back
Or if they need to adjust to the tank Conditions.
If your plants are dying in a cycle like every week.and then growing back
You need to call the X-files
