Conjoined Platy Fry

Hi

My female Platy gave birth a couple of days ago. i have 8 fry but the ninth one is actually joined together with another one. it has four eyes two tails and two bodies but joined at the stomack.

what is the cause of this ?
can this occure
and who do i report it to ?
