cleaning a fish tank

I know about cleaning tanks, water changes, clean the gravel.
My question is.... some tanks look very nice with lots of plants rocks etc.
How are they cleaned properly?
I cant see people taking out the rocks etc to cleam the gravel.
 
i dont vac the gravel near my plants, they use the dirt.

every month or so i remove all the rocks and wood and vac underneath tehm, because a surprising amount of stuff gets stuck under it. :/
 
I just vaccuum around my plants to get rid of all the gunk. It doesn't displace them at all.
 
I vacume and stir up some of the gravel or sand every time I do a water change which is once a week 10% or anytime the water looks cloudy
 
I would remove some of the rocks and plants every month or two to suck out the dirt that's gotten stuck.
But on weekly cleaning basis, I just go around the rocks and plants.
Not under them.
