Bala Shark Sick

Hi, i have a sick bala shark and im not sure what is up with it. I have 4 balas about 2inces long in a 48 gallon tank with a couple colombian silver sharks i resqued from my cousin. they seem to be getting along and doing well but yesterday i noticed one of my balas was just sitting there by itself and breathing really hard. about 3 times faster than the others. today i noticed he has lost a couple scales and there is a small whiteish patch above the scale loss area. kinda looks like a fungus but i dont want to treat it until im sure what it is. i have seperated it in a hospital tank i use for sick fish and am wondering what to do next. any suggestions?

1. Size of your tank?
2. What fish you have in it?
3. Water test results for ammonia, nitrite, nitrate and pH?
4. How long has the tank been set up?
5. How did you cycle the filter?
