I only use salt if I am treating a disease alongside standard medication. I do not believe it is neccessary, in fact some scaleless fish such as loaches and cories do not tolerate aquarium salt too well.
If you are using salt in your aquarium then remember that it won't evaporate from your tank and will only be removed in water changes, if you wish to maintain a level amount of salinity then yes you would have to replace at water changes.
Unless you are keeping fish that prefer a brackish water, and ONLY fish that prefer a brackish water I would keep the salt for medicinal purposes and let the freshwater fish live in freshwater...
