6 Year Old Gourami White Scale Patch On Head!

Hi

I would like some help into working out whats up with my 6yr old opaline gourami, All water parameters are ok nitrite nitrate amonia etc!

Since yesterday evening shes got a white flat scale patch on her head I was wondering is this a scrape or a infection? I haven't started treament yet befor im sure!

She has cloudy patches in both her eyes that are more central located and internal not over the film of the eye I thought maybe old age? I have attached some photos to help!

With the patch and cloudy eyes I think I would issolate the fish and use a bacterial med.

White patches on head can be inury, columnaris, parasites.

Any redness to the patch.

No no redness its shown up in the space of like 8 hours! its very flat almost indented and the size of one of her scales!
 
Has the fish got stuck in an ornament.
I think I would use a bacterial med.

Cloudy eyes are a symtom of a desease not a desease in there own right.
Causes.
Bad water quality.
Stress.
Old age.
Irratation.
Poor diet.
Injury.
Parasites.
Bacterial.
