hi, shall i leave the bottom of my hospital fry tank with no substrate or put a thin layer of gravel on the bottom

add some gravel. good for the bacteria, and good to hold plants and deco that the fry or sick fish can hide in.
 
personally i run quarantine/fry tanks without substrate as it's easier to keep them clean, with gravel the dirt is going to be hidden.

gravel holds litle to no bacteria with your standard filters as there's no direct flow going through it.

by all means add some kind of decor for hiding places of sick fish and maybe some realy plants for fry where they can gain some food from.
 
There best without substrate on the bottom.
