Update, she’s now swum back to the top of the tank and is sitting in her favourite plant like a clown fish, like usual. Really at a loss what to do but glad I didn’t call it earlier.
It’s now been lethargic for two months and last week I thought it was improving as it swam from one plant to another. Last night it actively ate from the surface. Today though she’s lying or sitting on the sand. It’s not well. Do I euthenise now or wait and let nature take its course?
She actually isn’t bloated, that’s a bad photo where a plant is cropping part of the body making it appear bloated. I hate it when I can’t help my fish, it’s been a long time since I’ve lost one.

Thanks, did a big water change and clean today (I do 40% change every 7-10 days) and finally the synodontis appeared this evening. Perfect shape, nothing wrong whatsoever! Whatever it was, it was something stuck to his side but not a growth. Needless to say, I’m very relieved.

Thanks for all the quick advice above, I’ve decided to feed 3 times a day over the next 4 days and do a water change on the day I go away. Hopefully all should be well.
I’m really surprised to read the comments about not leaving anything. I’ve been told that up to 10 days is ok to leave them unfed and I was planning a water change on the day I leave but felt I needed something in there food wise while I’m away.
