yes you need one.
you should do a gravel vac atleast once every 2 weeks
edit: what substrate do you have? if you have sand you will never have to vac.

i have sand and gravel..

mixed.. gravel at the back and sand at the front..

try and vacuum the sand, waste can lie on top, dont vacuum sand the same was as gravel though, sorta hoover the vacuum over the sand and stir very slightly in figure eights. If it is planted than no need to worry about vacuuming either unless it gets really crummy.
