Neverending Ich!

Current tank stats are: 55g 3 spotted SD's-14 black neon tetras-13 cardinal tetras-9 golden pristella tetras-4 albino cories-1 striped raphael catfish
-Temp 78 F
-PH 7.6
-Ammonia 0
-NitrIte 0
NitrAte 30-40

Tank has been set up and running for about 2 years. I perform a roughly 25% water change every week and haven't had any problems to speak of until a few months ago my fish got the ich. I added a SD to the tank after only 2 weeks in hospital tank.. Dumb move but i've learned.. Anyways so I noticed the ich really infested my cardinal tetras the most (I lost 2 during the first go round). So I treated with wardley wellness ick out sensitive (for the catfish) following the directions on bottle exactly how it reads.. So I removed the carbon and filters.. Vaccumed the gravel how they suggested and all that. They all ate great during the whole process and after treatment was over the fish looked great.. So 3 or 4 weeks pass and I notice the cardinals look salted again and the new SD had a few specks on his anal fin that weren't there b4.. So I decide to research online and go through old posts and come up with a game plan.. one thread really caught my eye where they were talking about treating ich without chemicals.. Raising temp to 86 F doing partial water changes/gravel cleanings over the course of 15-20 days as everyone had an opinion.. Most were between 15-20. So I went with 16 days and followed directions exactly.. Ich seemed to be eliminated after 16 days so I cleaned gravel one more time and started lowering the temp slowly back to 78F.. Once again 3-4 weeks later it's popping back up with the cardinals looking the worst again.. Am I going about this wrong? Even thou I have ich now for the 3rd time in 3 months I still consider this my first bout until I finally rid my tank of this pest.. Any suggestions? What could be causing this to return like bad zits? I am trying once more raising the temp and treating without chemicals.. Would treating this way for 21 days kill it once and for all or is that excessive? Should I go back to a chemical treatment? I need some HELP!!
 
Also does anyone know why my cardinal tetras look so horrible.. A few are totally covered with white specks yet some other fish don't have any noticable spots at all... Same thing when I was treating over a month ago. Cardinals look bad.. But eventually all spots are gone during the course of treatment.. I'm exausted with this. I need a foolproof reliable plan of action... Please help

Whitespot looks like the fish has been sprinkled in salt.
You have to raise temp and then add the med, increase aeration.
You still keep treating the fish one week after spots have gone.
Any red pin prick marks on the fish
What do the fish gillls look like.
