Damselfly or dragonfly larvae most likely. Those are the two most often encountered that fit that description.

It is most likely a insect larvae that came on one of the plants. You can try to stop these guys from getting in by rinseing your plants litely before placing them into your tank :good:

most likley a damselfly larvea from what you say .
are the three tails more like thick hairs?
i dont mean to worry you but some dragon fly larvea will reach at least 2 inches long and prey on fish the same size .
big eyes normally belong to a predator so keep an eye on that little critter.

i also do a bleach dip before placing plants into my tank however i think they all came from my planted tank except the java moss which i dipped in bleach

If it looks like this:

damsel_nymph.jpg

Then it's definitely a Damselfly Nymph.

"Some larger species have been known to feed on small fish."
