poorly guppy

Appeared overnight. What on earth is this on my guppies tail?

Ph 7.8 Ammonia 0 Nitrites 0 Nitrates 20 Mrs guppy hugs sitting on the substrate gasping. She was scratching her fills against the plants so I've treated the tank for gill flukes. All the other fish are happy. Any advice welcome. Thanks
