can have air blocked?

Hi there, Well, the fish issue got to my attention for about 1 month now. He looks troubled because of the belly but is summing, it's still active. I did not quarantine it, is not eating like before. It's poop I don't know.. can be congested.

I tried to change the water, now I fast him for about 2,3 days but it is not improving. He is still active and will eat here in there but I'm very concerned. Please have a look at the picture. Any ideas about what can be and if is treatable?
