Yes the substrate is very smooth and the driftwood you see in the picture is very soft. There is maybe 1 rough rock in the tank but he never really hangs out near it. I gravely vac about once a week as best I can without knocking over all their hiding spots, then maybe once every 2 months or so...

Thanks for the tips, I think for now I'm just going to get the tank stable enough to put return my snail to then put the pleco in my sick tank and start antibiotics to see of that does the trick.
